The physical exam is a good opportunity to review all required immunizations with your doctor.<\/p>\n<p>&nbsp;<\/p>\n<h4>Immunizations<\/h4>\n<p class=\"p1\">Immunization information must be completed and immunizations up-to-date in order for your health record to be considered complete. Some majors (i.e. Physical Therapy) may require additional entrance requirements. Please inquire directly with the Clinical Education coordinator or your advisor about requirements.<\/p>\n<p class=\"p1\">LVC requires the following immunizations:<\/p>\n<ul class=\"ul1\">\n<li class=\"li2\"><span class=\"s2\">TDAP\/Adacel\/Boostrix\u00a0<\/span>(booster within last 10 years)<\/li>\n<li class=\"li1\">Meningitis vaccine at or after age 16 (Meningococcal conjugate, MCV4, MenACWY, Menactra, Menveo, MenQuadfi)<\/li>\n<\/ul>\n<p class=\"p1\">The following immunizations are recommended, but not required:<\/p>\n<ul class=\"ul1\">\n<li class=\"li1\">Hepatitis A<\/li>\n<li class=\"li1\">HPV<\/li>\n<li class=\"li1\">Meningitis B<\/li>\n<li class=\"li1\">Öйú×ã²ÊÍø<\/li>\n<li class=\"li1\">OPV (polio) series<\/li>\n<li class=\"li2\"><span class=\"s2\">MMR\u00a0<\/span>(2 doses required)<\/li>\n<li class=\"li2\"><span class=\"s2\">Varivax vaccine\u00a0<\/span>(2 doses required<span class=\"s2\">) or Varicella Disease\u00a0<\/span>(date\/age of disease is required)<\/li>\n<li class=\"li2\"><span class=\"s2\">Tuberculosis Testing (PPD\u00a0<\/span>skin test<span class=\"s2\">) within the past 12 months\u00a0<\/span>(IGRA\/Quantiferon gold blood test is acceptable)<\/li>\n<li class=\"li2\"><span class=\"s2\">Hepatitis B\u00a0<\/span>(series of three)<\/li>\n<li>Influenza<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<h4>Additional Forms<\/h4>\n<p>If you have any documented medical\/mental health needs that require special accommodation, please take the time to fill out the form(s) that is most relevant for you:<\/p>\n<ul>\n<li><a href=\"\/housing-accommodation-form\" target=\"_blank\" rel=\"noopener\">Housing Accommodation Verification Form<\/a><\/li>\n<li><a href=\"https:\/\/www.metzlvc.com\/allergies.html\" target=\"_blank\" rel=\"noopener\">Food Allergies &amp; Sensitivities Form<\/a><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<h4>Health Insurance Coverage<\/h4>\n<p>Lebanon Valley College requires all residential international students to maintain medical insurance that provides coverage in the United States. Therefore, enrollment in the Lebanon Valley College-sponsored health insurance plan is automatic and the cost of the policy is included in the student\u2019s tuition and fees invoice. In certain cases, students holding comparable insurance policy coverage may be eligible to waive enrollment in the LVC&#8217;s health insurance plan. In addition to personal health insurance coverage, the comparable plan must include the following benefits and minimum limits in order to be considered for an exemption:<\/p>\n<div>\n<div>\n<div class=\"wysiwyg__tableWrapper\">\n<table style=\"height: 98px;\" role=\"presentation\" width=\"800\">\n<tbody>\n<tr>\n<td style=\"width: 259.333px; text-align: center;\" colspan=\"3\">International Students Attending Classes at Lebanon Valley College<\/td>\n<\/tr>\n<tr>\n<td style=\"width: 259.333px;\">Emergency Accident<br \/>\n&amp; Sickness<\/td>\n<td style=\"width: 259.333px;\">$100,000<\/td>\n<td style=\"width: 259.333px;\">Can be included in personal health insurance if coverage extends to care in the United States.<\/td>\n<\/tr>\n<tr>\n<td style=\"width: 259.333px;\">Medical Evacuation<\/td>\n<td style=\"width: 259.333px;\">$250,000<\/td>\n<td style=\"width: 259.333px;\"><\/td>\n<\/tr>\n<tr>\n<td style=\"width: 259.333px;\">Repatriation<\/td>\n<td style=\"width: 259.333px;\">$50,000<\/td>\n<td style=\"width: 259.333px;\"><\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<\/div>\n<\/div>\n<\/div>\n<p>&nbsp;<\/p>\n<p>Additionally, students will provide evidence that the comparable policy has a health insurance claims office based in the United States. Students requesting a waiver review of their individual insurance policy should submit the following information to <a href=\"mailto:international@lvc.edu\">international@lvc.edu<\/a> by <strong>Aug. 1<\/strong> for the fall semester and <strong>Dec. 1<\/strong> for spring semester:<\/p>\n<ul>\n<li>Insurance company name<\/li>\n<li>Insurance company phone number and claims address<\/li>\n<li>Policy number<\/li>\n<li>Policyholder name and date of birth<\/li>\n<li>Photocopy of policyholder ID (if already purchased)<\/li>\n<li>Detailed list of coverage benefits. The detailed list should be specific to your individual plan. A generic list from the insurance company\u2019s website is insufficient. Please contact your insurance directly to obtain your detailed list of coverage.<\/li>\n<\/ul>\n<p>After review and acceptance of your submitted waiver form, your Lebanon Valley College tuition invoice will be credited with the automatic policy premium.
